Amend markup and style for new menu table

- Restore ID for CSS
- Use semantic <label> instead of <span>
- CSS into default style file instead of inline (except for width=1 trick
to shrink first column)
- Fix default style to align with rest of menu blocks
- Move 'Use' button to its own row
